Ultrasound was not used for imaging of the human heart until 1954, but from this time there has been tremendous progress in ultrasound technology and its application for examining the heart and great vessels. There is no doubt that the introduction of echocardiography into clinical practice represents one of the most important medical achievements of the past century. However, in the early days of echocardiography, the quality of transthoracic images was poor compared with present standards, and cardiologists often encountered a patient whose chest was impenetrable to the ultrasound waves. Thus, it is not surprising that they looked for a new window onto the heart that was not obscured by the air in the lungs or bony structures of the chest wall. Finally, in 1975, Dr Lee Frazin discovered this window in the oesophagus

Since cardiologists were not always available to perform the diagnostic TOE procedures in the operating room, the opportunity for a new role as an echocardiographer emerged for anaesthetists and, subsequently, many of them took it. In the early days of TOE, this created a violent controversy within the anaesthesiology community. The "anaesthetist echocardiographers" were called irresponsible "cowboys" and asked to "keep the scopes in their holsters". Fortunately, these times are overcritical anaesthesia and at the same time perform TOE. One should be aware that TOE as a monitoring and diagnostic tool is an integral part of anaesthetic management, particularly in critical situations. In our opinion it is, in most cases, appropriate that the anaesthesia team be responsible for both anaesthesia and TOE. However, there will always be situations when assistance from a cardiologist will be needed and welcome. This approach is common practice in our institutions

In 1990, the Association of Anaesthetists of Great Britain and Ireland(AAGBI) published its first report on suspected anaphylactic reactionsassociated with anaesthesia.
In 1995, a second edition was published jointlywith the British Society for Allergy and Clinical Immunology (BSACI) in anattempt to help members of both societies with advice about theinvestigation and subsequent management of patients suspected of havingsuffered an anaphylactic reaction associated with anaesthesia.
The third working party was established to give particular attention to theresults of recent reports such as that of the Resuscitation Council dated1999, and on the value of investigations and their availability to anaesthetistsin Great Britain and Ireland.
The incidence of anaphylactic reactions and the associatedmorbidity/mortality in the UK is still unclear. It is the view of the workingparty that the incidence of suspected anaphylactic reactions duringanaesthesia is low but the number of cases may be increasing.2.4
There remains uncertainty about which laboratory investigations should beundertaken following a reaction as well as their interpretation andsignificance.
The role of the anaesthetist in the investigations has been acause for concern.

Description: Some important constraints of anesthesia must be taken into consideration when the pharmacological properties of modern anesthetics are discussed. The most important of these could be that the target effect be achieved preferably within seconds, at most within a few minutes. Similarly, offset of drug action should be achieved within minutes rather hours. The target effects, such as unconsciousness, are potentially life-threatening, as are the side effects of modern anesthetics, such as respiratory and cardiovascular depression. Finally, the patient’s purposeful responses are not available to guide drug dosage, because, either the patient is unconscious, or more problematically, the patient is aware but unable to communicate pain because of neuromuscular blockade.
